Annual Health and Medical Record

Only* Part A and Part B (both pages) must be completed by every Cub Scout, Adult Partner, and Scouter (adult leader). Please return these forms at the next pack meeting. Visit BSA’s FRQs Concerning Annual Health and Medical Record website for more information.

*Part C is only required for participation in events lasting longer than 72 hours like the Webelos & Cub Resident Camp at Camp James Ray.

Uniform and Handbook

Check out BSA’s Cub Scout Uniform requirements or consult with your Den Leader or Cubmaster for guidance. Generally speaking, Pack 443 Cub Scouts wear the uniform to indoor den and pack events. Otherwise, we wear our Pack 443 activity shirts.

You may purchase your scout’s uniform online at Scout Shop, but it’s easier if you visit one of the local scout shops: 

    • Region 10 Scout Shop in Fairview (5600 US Hwy 75 South, Fairview, TX, 75069)
    • Lewisville Scout Shop (2305 TX-121 #180, Lewisville, TX, 75067)

These shops offer sewing services 🙂

Besides the uniform, please obtain a handbook to track your scout’s progress through the year.
